Project Purpose
To ensure older youth aging out of foster care have the supports to enhance adult self-sufficiency in the community.

Project Eligibility
Youth age 18-20 exiting the foster care system or who have left foster care and have not yet reached age 21.
Project Referral Protocol
E-mail Chafee@hsc.wvu.edu with:
- Youth’s Name
- DHHR Worker Name and Phone Number
- Referent Name and Phone Number
- Expected Date of Discharge from the Foster Care System
