IMPACT WV wants to hear from families with young children impacted by substance use

Take the anonymous Family Needs Assessment Survey

The IMPACT WV program is seeking participants for a short online survey to guide trainings and resources designed to help pregnant women and mothers who currently use or have used substances in the past.

This survey is fully anonymous and there is no risk invovled with participating. It should only take about 10 minutes to complete.

Before you start the questions, please keep in mind that:

  • You can work on this survey when you get the chance; 
  • Stop when you need to; and  
  • Come back to it to finish when you can.         
  • If some questions make you uncomfortable, or you do not want to answer, skip them.       

Please remember this survey is for anyone who fits into one of these groups:

  • Pregnant women or mothers who have misused at least one substance and have a child under the age of 5.
  • Family member of persons who have someone in their home who is currently (or has) used substances and have a child under the age of 5.
