Have you received a service from the CED? We want your feedback!
At the WVU CED, we want to continue to provide services that are helpful and effective for you. To make sure we meet this goal, we are asking for feedback if you, or a member of your family has received services from the center in the past few years.
Please take the short survey to help us identify how you prefer to receive services and what additional information you have found would be beneficial to you. The survey is expected to take no more than 10 minutes.
Please complete the survey if you have received any helpful guidance, training, or other services from the CED. A parent, guardian, or caregiver of someone who has received these services may also complete the survey based on their experience and needs.
